I watched the movie "Adaptation" (Starring Nicholas Cage) last night... It was "pretty good". I think Cage over-acted the part a bit. The protagonist has a fear of social situations, and spends all of his time second-guessing things he does, and what people thinka bout him, and it ends up paralyzing him socially so that he can't participate normally in society.
